New Patient Information

In preparation for your child’s first appointment:

  • We find it’s often helpful to obtain a prescription/order from your pediatrician for occupational therapy evaluation and treatment, although we can begin services without one. Some insurance companies do require prescriptions for OT and others do not. Our billing team can help find this information for you. A referral is different from a prescription for OT and if you have questions please feel free to email our office manager Alayna at alayna@canyonkids.com.

  • We will send you a link to join our secure online portal. There, you will be able to conveniently fill out our intake forms prior to your scheduled evaluation and submit them directly to us. Our forms can be completed on a phone or desktop.

  • If your child has received prior evaluations from other professionals these can be uploaded to the portal or emailed to alayna@canyonkids.com.

  • Some children are nervous for their first visit to our clinic. Showing them the virtual walk through of our space may help them to become more comfortable with our office prior to their first appointment.

  • We welcome you to accompany your child to their evaluation or session. Caregivers may also wait in our waiting area.

Referral Process and Insurance Policies:

Children may be referred to Canyon Kids via self referral, school referral or by a physician/other healthcare provider. Your insurance verification will typically note if an prescription/order or referral is required by your insurance company. We recommend all families check with their insurance provider if a prescription or a referral is required prior to beginning out of network occupational therapy services. We have found that Care First and Tricare plans always require a prescription for OT as well as a referral to Canyon Kids. We will submit claims to your insurance after each therapy session and follow those claims for you. It is always helpful to monitor your EOB’s and if you have questions contact your insurance company directly.

Questions to ask your Insurance Carrier:

  • Do you provide reimbursement for Out of Network Occupational Therapy?

  • Do you cover codes such as: 97530, 97167

  • Does my plan require a prescription from my PCP?

  • Does my plan require a referral from my PCP to Canyon Kids for OT services to be covered?
